Beginning August 1, 2017, all new ONEcards issued will be proximity cards. The ONEcard program will continue to cover the cost of the first card issued to staff members and students registered in credit courses; upgrades and replacement cards (lost, stolen and/or damaged cards) will still be subject to a fee.
The forms below are only required if a department representative is paying for the proximity upgrade or replacement using a University of Alberta speedcode. If the cardholder is paying the fee (or is receiving their first ONEcard), the forms below are not needed.
If you currently have a ONEcard please make sure to bring it with you to exchange for the new card
