With the implementation of Google Apps for Education at the University of Alberta, the ONEcard Office is attempting to move to authenticated (i.e. user must login with CCID) forms. Until we have this process completed, we will continue to provide a .pdf form that can be printed and signed before bringing it to the ONEcard Office. If you have any questions about the forms please contact us.

Proximity ONEcards

Prox, proxy or Proximity cards are a premium type of card used on the University of Alberta campus for door access at specific locations including Lister Centre, International House and an increasing number of Labs and other buildings.

Proximity ONEcards are available to anyone who is currently eligible for a ONEcard. If you are a current ONEcard holder, you can exchange your current ONEcard for a proximity ONEcard. There is a $15 fee for the upgrade, which can be paid by the cardholder at the time of issuance, or may be charged back to a department using the form below:

Please bring the completed form, along with your current ONEcard (to exchange), to the ONEcard office.

Department Badges

The ONEcard office also produces ID badges for a number of faculties and departments on campus.  The following form is designed to be completed by a department administrator or supervisor and then brought to the ONEcard office.

^NEW If you are an authorized approver for your department you may also complete the electronic version of our Badge Request Form. Please note, CCID authentication is required and your CCID will be recorded when submitting a form.

Other Forms

If you are not eligible for a standard ONEcard (i.e. not a current student, staff, alumnus, emeritus, post doc or academic visitor) and have never been issued an ID number you may submit the ONEcard/ID Request Form.
